To create an Upload Media question type:

Select Upload Media from the Drop Down Question Type

- Request Photo, Video, Document or all media types

Click on Preview to review your question

Click on Save to save and create your question

1. Go to the Questions Section
2. Click on Create New Question
3. Type your question
4. Select Upload Media from the Drop Down Question Type
 
 - Request Photo, Video, Document or all media types
5. Select your preferred Attributes
6. Click on Preview to review your question
7. Click on Save to save and create your question
