To create a Date and Time question type:
Go to the Questions Section
Click on Create New Question
Type your question
Select Date and Time from the Drop Down Question Type
Request Date, Time or Both information
Select your preferred Attributes
Click on Preview to review your question
Click on Save to save and create your question
